Cardiac potassium channel subtypes: new roles in repolarization and arrhythmia.

چکیده
About 10 distinct potassium channels in the heart are involved in shaping the action potential. Some of the K+ channels are primarily responsible for early repolarization, whereas others drive late repolarization and still others are open throughout the cardiac cycle. Three main K+ channels drive the late repolarization of the ventricle with some redundancy, and in atria this repolarization reserve is supplemented by the fairly atrial-specific KV1.5, Kir3, KCa, and K2P channels. The role of the latter two subtypes in atria is currently being clarified, and several findings indicate that they could constitute targets for new pharmacological treatment of atrial fibrillation. The interplay between the different K+ channel subtypes in both atria and ventricle is dynamic, and a significant up- and downregulation occurs in disease states such as atrial fibrillation or heart failure. The underlying posttranscriptional and posttranslational remodeling of the individual K+ channels changes their activity and significance relative to each other, and they must be viewed together to understand their role in keeping a stable heart rhythm, also under menacing conditions like attacks of reentry arrhythmia.

Role of abnormal repolarization in the mechanism of cardiac arrhythmia.
In cardiac patients, life-threatening tachyarrhythmia is often precipitated by abnormal changes in ventricular repolarization and refractoriness. Repolarization abnormalities typically evolve as a consequence of impaired function of outward K+ currents in cardiac myocytes, which may be caused by genetic defects or result from various acquired pathophysiological conditions, including electrical ...

Although work on class III antiarrhythmics remains at an early stage, these agents still appear to possess greater efficacy and less proarrhythmia than conventional class I agents in those experimental arrhythmia models considered to be most representative of the clinical situation. Although prolongation of repolarization carries with its own tendency for pause-dependent arrhythmogenesis (i.e.,...
متن کامل[A case of long QT syndrome].
Introduction Long QT syndrome (LQTS) is a group of genetically distinct arrhythmogenic disorders resulting in abnormal cardiac sodium and potassium ion channels causing delayed repolarization of the heart. This syndrome can present clinically as syncope, seizures, or sudden cardiac death secondary to its characteristic ventricular arrhythmia of torsades de pointes. The clinical and electrocardi...
متن کاملMechanism of Potassium Channel Block and Its Role in Suppressing and Aggravating Cardiac Arrhythmias
T he increased mortality associated with the use of the class Ic agents encainide and flecainide in the Cardiac Arrhythmia Suppression Trial (CAST)' has led to a critical reexamination of the adequacy of existing therapies for the control of cardiac arrhythmias. Although the reasons for the findings in CAST remain unclear, proarrhythmia due to excessive slowing of conduction has been suggested ...
متن کاملA place for high-throughput electrophysiology in cardiac safety: screening hERG cell lines and novel compounds with the ion works HTTM system.
Several commercially available pharmaceutical compounds have been shown to block the IKr current of the cardiac action potential. This effect can cause a prolongation of the electrocardiogram QT interval and a delay in ventricular repolarization. The Food and Drug Administration recommends that all new potential drug candidates be assessed for IKr block to avoid a potentially lethal cardiac arr...
